Cole Family Dental had done everything right up to the booking. The new patient funnel was working. The phones were ringing. The appointments were being made. Sixty-four new patients in a single month. That is not a small number for a six-chair practice.

Eighteen of them never showed.

Not because they changed their minds. Not because the practice did something wrong. Because nobody followed up. One text, sent two days before the appointment, was the entire relationship between booking and first visit. A new patient relationship does not begin at the first visit. It begins at the booking confirmation. The practice that treats those two moments as the same thing will always have no-shows it cannot explain.

The gap between a booked appointment and a kept appointment is a trust gap. The patient agreed to come in. They have not yet decided the practice is worth showing up for. That decision gets made in the 48 to 72 hours before the appointment, when they are either receiving thoughtful, timely communication from you or they are not hearing from you at all and quietly reconsidering.

One confirmation text sent 48 hours out does not close that trust gap. It acknowledges the appointment exists. That is not the same thing as making the patient feel the appointment matters. A three-touch sequence, booking confirmation plus 72-hour reminder plus 24-hour final confirmation with pre-visit instructions, gives the practice three moments to demonstrate that the relationship is real before the patient walks through the door.

The second failure here is structural. When a patient does cancel or no-show, the empty slot should be filled before it is ever noticed by the front desk. That requires a waitlist with an automated backfill trigger. Cole Family Dental had a waitlist. It was a spreadsheet. Nobody was watching it.
